Overview
Show an intake form that keeps an honest record of what you did, with this file upload panel for React with Ark UI. It is the intake page of Waitwell, a small clinic's records desk, and it is a layout only: it uses sample details, makes no claim about privacy, security or compliance, and says so in its header and its foot. A Segment Group picks the record type (Lab result, Imaging, Referral, Visit note), three Input fields take a fictional patient's name, a reference and the service date, and a single-file File Upload strip takes the file. Beside the form, a This session list writes down each action in order, with no clock: "1. Record type set to Imaging", "2. File chosen: lab-0312.pdf, 1.4 MB". Add record, the one Shine Button, checks the fields and says what is missing. The frame's bands are ruled by Guides. For intake desks, referral forms and any app that wants a visible trail of what was done on a page.
FAQ
Is this safe for patient records?
No claim is made. It is a layout with sample details, it has no storage, and the Layout only badge in the foot says so. Do not type a real patient's details into it. A real intake page needs the privacy, security and legal work of the place that runs it, which no block can supply.
What does the This session list record?
Each change you finish and each action you take, in the order it happened: the record type chosen, a field changed (written when you leave the field, not on every key), a file chosen, replaced or refused, a file removed, and every press of Add record with what was missing. It only ever adds to the end, keeps the last sixty and shows no time, because the list is an account of this tab, not an audit log.
Is the file read?
No. One file of any type up to 25 MB is listed by name and size, and replacing or removing it is added to the list. A file over the limit is refused with a polite line and a list entry.
What does Add record do?
It checks that the name, the reference, the date and a file are there. If something is missing it names it in the foot and marks the field; if not, it says "Record added in this demo. Nothing was uploaded or saved." Either way the press is added to the list.
What stays local, and how do I wire it to a server?
All of it. The fields, the file's name and the list are state in the block, gone on reload. In your app post the record from submit, write the real audit trail on your server with identities and times, and let this list show what the person did in the page.
